This agreement shall apply to all boarding, daycare, and board-and-train services provided by Dirigo Training & Boarding (the “Kennel” or “Trainer”) to you (the “Dog Owner” or the “Owner”) for your dog described below in this agreement. Please submit a new form for each dog. Please review our Policies prior to signing up for services. Drop-Off/Pick-Up times are by appointment. If the Dog Owner will not be able to arrive at the scheduled time, please let us know in advance to reschedule. Pick-ups must be done by 8 am to avoid boarding charges for that day. Drop-offs after 6pm will not incur a charge for that day. No day-of holiday drop-offs or pick-ups at this time (ex: no drop-offs or pick-ups on Christmas Day). Sunday drop-offs or pick-ups ONLY from 4pm - 7pm by appointment.

A recent document from your veterinarian is required showing your pet’s medical records for the past year. Please upload these documents below, or email them to [email protected] at least 48 hours prior to drop-off. Document must show dates of the following:

Current Bordetella vaccination (Kennel Cough) within the past 6 months
Current DHLPP vaccination (Distemper, Hepatitis, Parvovirus)
Current negative heartworm test result
Proof of current flea/tick/heartworm prevention
Proof of current rabies
Negative fecal test (GIARDIA, OVA, and PARASITES) within 90 days of your scheduled drop-off

This Agreement covers the current relationship between Dirigo Training & Boarding and myself. Each time I bring my dog to Dirigo Training & Boarding, I affirm the terms of the Agreement and the truthfulness and accuracy of all statements I make in this Agreement.
